An exciting and rewarding opportunity is available for a suitably qualified Registered Nurse to join the Complex Care team at Portland District Health.
The primary role is to ensure all consumers are receiving the highest level of clinical care by providing a level of decision making and contribution to ensure the effective and efficient operations of your unit/department. You manage an environment that is conducive to person centred care, which fosters a culture of engagement and partnership, whilst contributing to the quality, safety and risk management programs for the division of Primary Care Services. This role requires availability on Mondays and Wednesdays.
About you:
- Registered Nurse with Australian Health Practitioners Regulation Agency, and holds a current registration with a minimum of three years’ experience in relevant clinical area
- Post Graduate clinical qualification(s) in speciality area of practice or willingness to undertake further study
- Membership of an appropriate professional association
- Clinical knowledge, skills and leadership experience in nursing speciality of chronic and complex care coordination
- Demonstrated level of nursing skills, which is easily identified as a role model of clinical competence
- Understanding of relevant accreditation and funding systems, and demonstrated ability to manage continuous improvement portfolio
- Demonstrated understanding of the needs, issues and sensitivities of people from diverse backgrounds
- Understanding of opportunities and challenges associated with providing rural health care services
